How can I convert a card I possess into a shop item?

It costs 30K plus coins to convert a card into a shop item. To do this, you must donate 30K plus coins to @Stannie and then complete the respective form in the Forms section and post it in this thread.
Please note, you may only submit up to a maximum of 2 conversion requests a month.
